Forest Park HS Athletic Registration

Parents/Guardians of student-athletes intending to participate in athletics MUST upload their student's most recent physical to ensure eligibility for participation and complete the registration process. Registration cannot be completed by the student-athlete.

New VHSL Physical Form (04 2025)

The new form (04 2025) MUST be usde July 1 and after. Previous versions of the form cannot be accepted. Pages 1,2,3 will be handed in to school via Arbiter. The remaining pages will remain on file witht the physicians office.
